This locator can help you find the address of a local SSA office. Telephone numbers to local offices are not available. Please contact the toll-free customer service center for assistance.

Where can I get general information on Social Security benefits?

The Social Security Administration has established a central customer service center with a nationwide toll-free number to assist consumers with all types of Social Security questions. You can also visit or contact your local Social Security office.

Where do I get a duplicate Social Security Card?

There is no fee to get a duplicate card, and your duplicate card will have the same name and number as your previous card.

To use this free service you'll need to complete an application for a Social Security Card. You can get this form by calling, toll-free, the Social Security Administration's (SSA) central customer service center or by visiting your local Social Security office. You can download Form SS-5 from the SSA website at www.ssa.gov.

You will need to show evidence of your identity and show evidence of your U.S. citizenship (or lawful alien status if you were born outside the U.S.).

How do you change your name and address on your Social Security Card?

You will need to visit your local Social Security Administration Office to complete this process.

Be prepared to show proof of identity, such as a driver's license. The form you will fill out for this amendment is Form SS-5. It is available at your local office, or you can download it from www.ssa.gov.
